A logical name, or libref, is a name for the data library that you associate with an SPD Server domain during a SAS job or session. Once a libref is assigned, SPD Server allows you to read, create, or update files in the data library if you have the appropriate access to the data library. A libref is valid only for the current SAS job or session. Librefs can be referenced repeatedly during a valid job or session. SAS does not limit the number of librefs that you can assign during a session. Once you define a libref, it is most commonly used as the first element in two-level SAS file names: LibraryName.Tablename. The library name, or libref, identifies where the SPD Server can find or store the file. One can afford to be a lot less efficient in the use of inputs that he gets for free. Capital-intensive techniques that increase output per man-hour, but reduce output per acre, are suited to the interests of American-style agribusiness. They're perfect for large landowners who, as a historical legacy, have preferential access to large tracts of land to the extent that they can even afford to hold significant parts of it out of use ; , but want to reduce their dependence on hired labor. In areas with underutilized land and unemployed population, on the other hand, it makes a lot more sense to increase output per acre by adding labor inputs. And this is exactly the pattern that prevails in small-scale agriculture. Lapp found, in a survey of studies from around the world, that small farms were universally more productive--far more productive--per acre than large plantations. Depending on the region and the crop, small farms were from onethird to fourteen times more productive. The efficiency of small proprietors working their own land, compared to plantation agribusiness using wage or tenant labor, is analogous to that of the small family plots in the old USSR compared to the state farms. Plantation agriculture is able to outcompete the peasant proprietor only through "preferential access to credit and government-subsidized technology."91 Green Revolution techniques are very "efficient" indeed--but only given the artificial objectives of those who stole the land. The same general observations apply to agribusiness in the developed world. As Michael Perelman observes, the intensive raised bed techniques of early modern Europe compare quite favorably to the outputs per acre of today's mechanized agribusiness. Test Application Screening Prostate cancer screening remains controversial. Proponents point to the high incidence, absence of preventive agents, ease of screening, potential curability of organ-confined disease, and lack of effective therapy for advanced disease. Opponents tout the slow progression of many tumors, high number of false positives, overdiagnosis of indolent disease, and treatment side effects that lead to diminished quality of life. Since screening commenced in the late 1980s, incidence of metastatic disease has decreased, while the incidence of organconfined, moderately differentiated disease has increased.2This suggests screening has been effective in identifying clinically significant, potentially curable disease while reducing the incidence of incurable disease. Mortality has decreased as well, 3 but this reduction may be due to factors other than screening and early detection.4 Results from the first randomized controlled trial to show direct evidence that screening reduces prostate cancer mortality have been controversial.5 Two more studies addressing this issue are ongoing; their expected completion dates are between 2006 and 2010.6, 7 Pending additional data, the American Cancer Society8 and the American Urological Association9 have recommended that digital rectal exam DRE ; and total PSA be offered annually to men 50 years of age who have a minimum life expectancy of 10 years. First session with the somatically preoccupied patient, the physician should undertake a careful evaluation based on a historical understanding of the evolution of the patient's illness and health-related experiences to understand the process whereby the patient's illness has been organized. The way symptoms are disclosed to physicians is shaped by the social context within which the symptoms are experienced and to whom they are disclosed. An illness is organized through an evolving interaction between physician and patient.15 Information obtained from the history will assist in the recognition of patients with unreasonable fear of illness. The somatically preoccupied patient may present with a history of a greater number of outpatient visits, 16 more frequent hospitalization, 17 and repetitive subspecialty referrals18 than patients without these preoccupations. A medical record release may result in a "thick" medical record revealing multiple medications and a greater-thanaverage number of diagnoses and diagnostic studies. This type of record is a concrete manifestation of the somatically preoccupied patient who demonstrates a high utilization pattern. A past medical history of the following symptoms or syndromes has been associated with somatic preoccupation: chronic fatigue syndrome, fibromyalgia, irritable bowel syndrome, atypical chest pain, hyperventilation, pelvic pain, abdominal pain, low back pain, headache, dizziness, insomnia, and nonulcer dyspepsia.1921 A history of multiple drug allergies including extreme sensitivity to medication side effects may also indicate somatic preoccupation. A family history of anxiety and or depressive disorders might be another clue from the patient's history to indicate an excessive fear of illness. Noyes et al.22 observed.
