There is also increasing recognition that, indoor air quality problems are strikingly different across developed and developing nations. The most significant issue that concerns indoor air quality in rural environments of developing countries is that of exposure to emissions from combustion of biomass fuels and fossil fuels that are used for cooking and heating. Use of open fires for cooking and heating exposes an estimated 2 billion people in the world to enhanced concentrations of particulate matter and gases, up to 10-20 times higher than ambient concentrations. Although, biomass makes up only 10-15% of total human fuel use, since nearly half the worlds population cooks and heats their homes with biomass fuels, indoor exposures are likely exceed outdoor exposures on a global scale. Exposure to these pollutants has been shown in many recent studies to be causally linked to several health effects especially in women who cook with these fuels and young children. Strong associations between biomass fuel exposure and increased incidences of chronic bronchitis in women and acute respiratory infections in children have been documented. Recent estimates of the burden of disease attributable to use of biomass fuels in India puts this figure at 5-6 percent of the national burden of disease with acute respiratory infections in children under five contributing the most to annual health burden from indoor air pollution. Similar estimates for other countries too indicate that the disease burden due to indoor air pollution is high and often exceeds that due to outdoor air pollution. There are several challenges to accurately assess health risks for children from indoor air pollutants in developing country settings. Currently, limited quantitative information is available on household level concentrations and children's exposures, although recently conducted large scale quantitative assessments in India, Kenya and Guatemala are being used to create regional databases. Very few pollutants have been monitored and insufficient scientific evidence currently exists, to establish what may be good indicator pollutants for indoor air pollution. While most studies have been carried out in biomass using settings using respirable particulate matter as an indicator pollutant, there is need for additional information from coal using settings where contributions from gases such as SO 2 and heavy metals such as arsenic and fluorine may be substantial. Other settings such as schools homes located adjacent to roads with high vehicular emissions, urban rural schools using biomass fuels to cook the afternoon meals to feed children ; pose additional challenges in assessing exposures.
